Abstract 文献概要

　広角赤外螢光造影により，特発性頸動脈海綿静脈洞瘻の4例4眼について，静脈圧亢進時の脈絡膜循環を検索した。全例に脈絡膜静脈の選択的拡張と渦静脈の狭小化があった。一部の渦静脈は消失し，残った渦静脈は膨大部が消失し，周辺へ向かう静脈が拡張していた。長期観察例では，当初あったびまん性静脈拡張が7週後には選択的な脈絡膜静脈拡張となり，その静脈の一部は，ヘアピン状の走行をとって乳頭方向に向かった。経過とともに拡張した静脈の数が減少し，血流路の変化が続いた。造影初期には，渦静脈から後極へ向かって拍動性に静脈が逆流した。乳頭周囲には，4眼中3眼で脈絡膜血流の新流出路が形成された。7か月にわたる長期観察例では初診の15週後から，周辺部に脈絡毛細管板の閉塞と考えられるベール状螢光の欠損部が多発した。持続的な静脈圧亢進により，脈絡膜静脈の選択的な拡張，血流路の変化，乳頭周囲での新流出路の形成などを内容とする一連の反応が起こることが示された。

Purpose: To evaluate modification of choroidal blood flow following elevated venous pressure in the orbit.

Material and Method: Four cases of carotid cavernous fistula with unilateral ocular manifestations were followed up by routine clinical methods and indocyanine green (ICG) angiography.

Findings: Initially, all the eyes showed selective dilatation of choroidal veins. Vortex veins were attenuat-ed, particularly at the ampullar portion. During the course of observation, the dilatation of choroidal veins became more selective. The direction of choroidal venous flow was reversed, running posteriorly towards the posterior fundus. Some choroidal veins were bent like hairpins near the optic disc. New sites of choroidal venous drainage routes developed near the optic disc in 3 eyes.

Conclusion: Sustained elevation of orbital venous pressure induced selective dilatation of choroidal veins, altered direction of blood flow and creation of new routes of venous outflow near the optic disc.
