超広角赤外螢光造影による糖尿病性派絡膜症の検索

Detection of diabetic choroidopathy by panoramic infrared angiography

　正常眼脈絡膜血管構築と糖尿病性脈絡膜症の検索を目的として，正常眼12眼と糖尿病網膜症47眼に対して，走査型レーザー検眼鏡によるインドシアニングリーン（以下ICG）赤外螢光眼底造影を施行した。画角40°のビデオプリントを貼り合わせ，超広角パノラマの脈絡膜造影図を作成して検討した。

　正常眼では，脈絡膜静脈の走行は，乳頭を通る水平および垂直線で区切られる領域から開始され，渦静脈に流入した。渦静脈は通常5〜10個存在した。赤道部では眼底を放射状に走行する動脈像が観察された。脈絡毛細管板はびまん性のベール状螢光として検出され，その螢光は後極部で強く周辺部では弱かった。造影後期になると眼底全域で脈絡膜間質へ弱いICG漏出が起こった。糖尿病網膜症では，脈絡膜静脈のコントラスト低下，脈絡膜静脈数の減少，狭細化，脈絡膜動脈壁の色素染，局所的な色素漏出，脈絡毛細管板の部分的閉塞が観察された。

　本方法は，糖尿病性脈絡膜血管障害の解明に有用であると結論された。

We evaluated 47 eyes with diabetic retinopathy by panoramic composite infrared angiography using indocyanine green and scanning laser ophthalmoscope. All the cases were non-insulin dependent and included 6 eyes with rubeosis iridis, 22 with proliferative retinopathy and 9 with simple retinopathy. The diabetic eyes were characterized by decreased contrast of choroidal veins, decreased number of visible choroidal vessels, attenuation of veins, dye staining of choroidal arteries, focal dye leakage and patchy areas of obliterated choriocapillaris. These findings were absent in 12 normal eyes and were interpreted as indicating the presence of choroidopathy in diabetic eyes.
