Purpose: To evaluate modification of choroidal blood flow following elevated venous pressure in the orbit.

Material and Method: Four cases of carotid cavernous fistula with unilateral ocular manifestations were followed up by routine clinical methods and indocyanine green (ICG) angiography.

Findings: Initially, all the eyes showed selective dilatation of choroidal veins. Vortex veins were attenuat-ed, particularly at the ampullar portion. During the course of observation, the dilatation of choroidal veins became more selective. The direction of choroidal venous flow was reversed, running posteriorly towards the posterior fundus. Some choroidal veins were bent like hairpins near the optic disc. New sites of choroidal venous drainage routes developed near the optic disc in 3 eyes.

Conclusion: Sustained elevation of orbital venous pressure induced selective dilatation of choroidal veins, altered direction of blood flow and creation of new routes of venous outflow near the optic disc.
