高安病の赤外螢光眼底造影所見

Indocyanine green angiography in Takayasu disease

　走査レーザー検眼鏡（SLO）により，高安病5例5眼で広角赤外螢光造影を行い，脈絡膜循環を観察した。造影初期は後極部，静脈相に入ってから周辺部循環を観察した。また，パノラマ眼底像を作成し，眼底全域の脈絡膜循環動態を検索した。その結果，5例中3例に脈絡膜循環異常が存在した。網膜血管吻合期の1例では，著しい脈絡膜循環遅延があり，脈絡膜動脈は拡張し，起始部から周辺まで明瞭に観察できた。また，脈絡毛細管板の螢光輝度は眼底全体で低く，充盈欠損部が多発し，脈絡膜静脈は全体に狭窄していた。毛細血管瘤期の2例中1例では，脈絡毛細管板の斑状の充盈欠損があり，他の1例では循環遅延のみで，形態的な変化はなかった。慢性的に動脈圧が低下する高安病では，脈絡膜循環遅延だけでなく，形態的変化が起こりうることが証明された。これは脈絡膜血管の反応様式を知る重要な知見であると判断された。

Indocyanine green (ICG) angiography was performed in 5 eyes of 5 cases of Takayasu disease. We used a scanning laser ophthalmoscope and a wide-angle montage technique. Three eyes showed abnormal choroidal circulation including retarded arterial flow, dilated arteries and attenuation of choroidal veins. Generalized hypofluorescence, filling delay or defect of the choriocapillaris was another frequent feature. The findings show that chronic arterial hypotension in this disease induces pathological changes in the hemodynamics and morphology of the choroidal vascular system.
