要約 目的：オルソケラトロジー（オルソK）の目的でミラクルレンズ®を使用し，角膜潰瘍を生じ視力障害を残した症例の報告。
症例：24歳，男性。10年前より近医にてミラクルレンズ®によるオルソKを開始した。オルソKを開始した当初は定期的な外来受診を継続していたが，最終受診は2年前であった。左眼疼痛，角膜混濁に伴う視力低下を自覚し，前医を受診し，角膜感染症の診断となり，2日後に当院を紹介され受診となった。細菌感染性角膜潰瘍の診断にて，同日より緊急入院した。抗菌薬頻回点眼，散瞳薬点眼，抗菌薬，ステロイド内服，抗菌薬点滴より加療を開始した。約2週間の入院加療にて感染傾向は消失したものの角膜上皮下混濁が残存していた。その後約6か月の経過で中央部角膜上皮下混濁が残存し，視力（0.3）にとどまっている。
結論：オルソKは，安全性の確認の取れている承認レンズの使用が求められる。また，レンズの適正使用の徹底した指導，外来管理が必要である。
Abstract Purpose：To report a case who developed persistent visual impairment due to corneal ulcer after wearing Miracle lens® for orthokeratology.
Case：A 24-year-old male was referred to us for ocular pain and impaired vision in the left eye since 2 days before. He had been wearing Miracle lens® for orthokeratology since 10 years before.
Findings and Clinical Course：Corrected visual acuity was 1.2 right and hand motion left. The left eye showed signs of bacterial corneal ulcer. He received treatments with topical and systemic antibiotics, and systemic corticosteroid. Inflammation in the left eye subsided 2 weeks later leaving subepithelial corneal opacity. Corrected visual acuity has been 0.3 left.
Conclusion：The present case shows that prolonged wear of contact lens for the purpose of orthokeratology may induce corneal ulcer with persistent visual impairment.
