Abstract Purpose: To report a child who developed corneal ulcer after use of contact lens for orthokeratology.

Case: An 11-year-old girl was referred to us for pain in the right eye since one month before. She had been receiving orthokeratology for myopia since 18 months before.

Findings: Visual acuity was hand motion in the right eye and 1.2 in the left when corrected by −4.25D=cyl−0.5D 15°. The right cornea showed an ulcer in its center associated with hypopyon. She was diagnosed with bacterial corneal ulcer. Culture of corneal scraping was negative. After treatment with topical and systemic antibiotics, the right visual acuity improved to 0.6 2 weeks later. The right eye showed persistent subepithelial corneal opacity and photophobia.

Conclusion: This case illustrates that corneal infection may develop as complication of orthokeratology.
