Abstract Background:Used for scleral buckle surgery, Miragel® is known to cause various late complications due to postoperative swelling and degeneration. Here we examined five cases that required Miragel® removal in the past 20 years at our hospital.

Subjects and methods:For these five patients who underwent buckle removal for complications thought to be caused by Miragel® swelling, we studied the period after scleral buckle surgery, symptoms before removal surgery, and complications associated with removal surgery. We then examined whether the symptoms improved after removal surgery.

Results:The patients were 58-76 years of age;three ware man and two were women. The period from scleral buckle surgery to buckle removal surgery was 20 to 36 years. Preoperative symptoms included ocular motility disorder(4 cases), increased intraocular pressure(2 cases), ptosis(1 case), eyelid swelling(1 case), and eyelid mass(1 case)(with overlapping symptoms). Complications associated with the removal surgery were postoperative low intraocular pressure and postoperative choroidal bleeding. In addition, in cases in which glaucoma filtration surgery was required after buckle removal, expulsive hemorrhage occurred during filtration surgery. The symptoms improved after the removal surgery in 3 cases, but the 2 cases in which the symptoms did not improve showed increased intraocular pressure.

Conclusion:For complications caused by Miragel®, symptom improvement can be expected with removal surgery. However, there is a possibility of complications associated with removal surgery, and it is important to make a treatment plan with this in mind.
