Abstract Purpose:To report a case who developed vitreous hemorrhage following laser vitreolysis and who was treated by vitreous surgery.

Case:A 51-year-old male was referred to us after sudden visual failure following the third session of laser vitreolysis for vitreous floaters in the left eye by a local ophthalmologist. He then received intravitreal injecton of SF6 gas but his vision failed to improve. He was seen by us 4 days after the incident.

Findings and Clinical Course:Corrected visual acuity was 1.0 right and hand motion left. The left eye showed massive vitreous hemorrhage. Vitreous surgery was performed the following day. Submacular hematoma was present posterior to vascular arcades. A white patch was present 3 mm away upper temporal to the disc and was presumed to be the result of laser irradiatioinal visual acuity remained 0.1 due to macular hole formation.

Conclusion:This case illustrates that laser vitreolysis may induce retinal irradiation with grave subretinal vitreous hemorrhage.
