オルソケラトロジーでブレスオーコレクト®を学童に使用し角膜潰瘍を発症した1例

Corneal ulcer in a child after orthokeratology

要約　目的：オルソケラトロジー（オルソK）用コンタクトレンズであるブレスオーコレクト^®使用中に角膜潰瘍を発症した症例の報告。

症例：11歳，女児。1年半前よりオルソKを開始し，適切なレンズケアを行っていた。3日前より右充血，眼痛を認め近医を受診し，改善しないため当院へ紹介となる。初診時視力は右手動弁，左（1.2）。右眼毛様充血，角膜中央部に類円形角膜潰瘍および前房蓄膿を認めた。細菌性角膜潰瘍を疑い，抗菌薬点滴，点眼で加療した。病巣擦過物などの細菌培養は陰性であった。角膜潰瘍は消退し，視力（1.0）に改善したが角膜上皮下混濁は残存した。

結論：角膜潰瘍は，レンズを正しく使用していても発生するオルソKの最も注意すべき合併症である。

Abstract　Purpose: To report a child who developed corneal ulcer after use of contact lens for orthokeratology.

Case: An 11-year-old girl was referred to us for pain in the right eye since one month before. She had been receiving orthokeratology for myopia since 18 months before.

Findings: Visual acuity was hand motion in the right eye and 1.2 in the left when corrected by −4.25D＝cyl−0.5D 15°. The right cornea showed an ulcer in its center associated with hypopyon. She was diagnosed with bacterial corneal ulcer. Culture of corneal scraping was negative. After treatment with topical and systemic antibiotics, the right visual acuity improved to 0.6 2 weeks later. The right eye showed persistent subepithelial corneal opacity and photophobia.

Conclusion: This case illustrates that corneal infection may develop as complication of orthokeratology.
