Abstract Purpose:To report a patient of iris tumor metastatic from sigmoid colon cancer diagnosed by the aid of tumor marker in the aqueous.

Cases:A 60-year-old male presented with iridociliary mass in the right eye. He had been diagnosed with sigmoid colon cancer with metastases to the lung, liver and lymph nodes 22 months before.

Findings and Clinical Course:A yellowish mass, 3 mm by 5 mm, was located posterior to the iris in the temporal sector. Biopsy of iridociliary tumor could not be performed due to poor systemic condition. Radiation to the right eye proved to be futile and was followed by increase of iridociliary tumor. The aqueous humor showed elevated level of CEA, a tumor marker, to 553.7 ng/ml 35 days after his initial visit. This finding led to the diagnosis of iridociliary tumor as metastasis from sigmoid colon cancer.

Conclusion:Measurement of tumor marker in the aqueous led to the diagnosis of iris tumor metastatic from sigmoid colon cancer.
