Abstract Purpose:To report 3 cases who developed endophthalmitis after insertion of Baerveldt® glaucoma implant.

Cases:The first case, a 50-year-old male, developed glaucoma in the right eye secondary to atopic dermatitis. The affected eye received intraventicular insertion of BGI. The implanted tube exposed 6 months later with intraocular inflammation. Conjunctival suture and vitrectomy were performed while preserving the BGI. There has been no recurrence for 6 years with vitreous acuity of 0.06 and intraocular pressure(IOP)of 13 mmHg. The second was a 69-year-old male who received BGI in the left eye for neovascular glaucoma and diabetic retinopathy. Tube exposure and endophthalmitis occurred 2 months later necessitating removal of BGI. The infection subsided but elevated IOP continued and resulted in blindness. The third case was a 36-year-old male with atopic dermatitis and with high IOP in the both eyes. Multiple surgeries were performed on both eyes. The left eye received two sessions of BGI insertion in the anterior chamber, followed by exposure of the tube and endophthalmitis. There has been no recurrence after vitrectomy and insertion of BGI in the posterior chamber. After 4 years, IOP stabilized at 23 mmHg with visual acuity of counting fingers at 20 cm.

Concluson:Endophthalmitis took various courses after removal of BGI. Infection at the site of tube exposure was suspected in 2 cases with past history of atopic dermatitis.
