　Bartter症候群は1962年にBartterらが初めて報告した二次性アルドステロン症の一種である．病因としては，プロスタグランジン（PG）過剰産生説，C1再吸収障害説などが報告されているが，その病態は今なお完全には解明されていない．以前から本症候群に対しin—domethacinが治療薬として用いられているが，indomethacin有効例では腎由来のPGと考えられている尿中PG-Eが上昇していることが多く，尿中PG-Eが正常な症例に同薬が有効であった報告は極めて少ない．今回われわれは尿中PG-Eが正常であるBartter症候群の1例を経験し，試験的に投与したindomethacinにより血漿レニン活性，アルドステロン濃度の改善を認めた．本症候群では尿中PG-E値にかかわらずindomethacin療法を試みるべきであると考えられた．

Bartter's syndrome, which was first reported by Bart-ter in 1962, is a kind of secondary aldosteronism. As the cause of this syndrome, some hypotheses such as hyper-production of prostaglandin, impaired chloride reabsor-ption are suggested and correlation of many factors is also considered. It has been reported that urinary pros-taglandin (PG) E level is high in indomethacin effective cases. There are few reports of indomethacin effective cases with normal urinary PG-E level. Our patient with Bartter's syndrome, whose urinary PG-E was within normal level, has shown improvements in plasma renin activity and plasma aldosterone concentration under indomethacin therapy. We suggest that indomethacin should be administered in patients with Bartter's syn-drome regardless of urinary PG-E.
