肥満者の肺機能に関する研究

Studies on Pulmonary Functions: in Obesity

1．緒論

　肥満者が「息切れ」を訴えることは臨床医のしばしば経験する事実であるが，その愁訴を招来する生理学的機序に関しては未だ十分な解析がなされてはいない。

　著者らは慶応義塾大学病院における短期入院特別健康診断，すなわち「人間ドック」の諸検査の一部として肺機能検査を行つてきた。人間ドックにおける被検対象は41才以上のいわゆる中・高年者が多く，また肥満を伴う症例が少なくない。かかる症例の肺機能検査成績に基いてその肺機能障害を診断し，レ線所見などと対照しつつ肺機能障害を来たした原因を検討してみると従来肺機能障害因子としてとりあげられていた気腫化，線維化，胸膜胼胝形成，無気肺化，あるいは気管支拡張などの病変によるものとしては説明し得ない場合が少なくない。例えばかなり高度の拘束性障害が認められるにも拘わらずレ線上胸胼膜腓胝はみられず，胸廓の変形も必ずしも高度とはいい難い場合がしばしばある。このような場合には一次的な肺・胸廓の病変ではなく，他のなんらかの原因により二次的に肺機能障害を来たしたものと考えざるを得ない。

1. The pulmonary function in 144 obese patients is presented.

2. The changes in vital capacity, RV/TLC ratio, maximum breathing capacity, ventilation reserve, timed vital capacity, distribution indicis observed and calculated, and in emptying rate are discussed with referrence to the degree of obesity.

3. The involvement of pulmonary function is classified into 4 types and the percentage in every types of involvement found in the obese patients is discussed.

4. It is concluded that the obesity is one of the important factor to cause the restrictive involvement of pulmonary function.
