要旨　早期胃癌の治療法の進歩によって切除範囲が可能な限り小さく設定されるようになり，より厳密な境界診断が必要となってきている．IIb型早期胃癌（表面平坦型）は正常粘膜にみられる凹凸を超えるほどの隆起・陥凹が認められないものと定義されており，側方範囲診断を行うには慎重を要すると考えられる．白色光を用い，画像処理により内視鏡医が注目する波長の内視鏡画像を作成する分光内視鏡であるFICEは，画像強調観察の中のデジタル法として分類され，内視鏡医は任意の条件設定を行うことにより診断に有用な内視鏡画像を得ることが可能であり，拡大観察においては微小血管や粘膜微細模様の明瞭化が可能である．IIb型早期胃癌においても，FICE拡大内視鏡観察を行うことにより，高い精度で簡便で非侵襲的に側方範囲診断を行えるものと考えられる．

　The preoperative identification of the horizontal extent of an early gastric cancer is essential for carrying out minimal invasive surgery. Early gastric cancer type 0IIb is defined as a lesion devoid of any elevation or depression exceeding the extent of irregularity as recognized in the normal gastric mucosa. Flexible spectral Imaging Color Enhancement（FICE）is a digital method of Image-Enhanced Endoscopy which can be used to gain insight into the pathology of the lesion by using spectral estimation technology. The FICE setting can be customized by the endoscopist and this system can more clearly visualize both microvascular architecture and microsurface structure than does white light endoscopic observation. The FICE system is non-invasive and makes it easy to determine margins of 0IIb type early gastric cancers.
