はじめに

　後縦靭帯骨化症（ossification of the posterior longitudinal ligament：OPLL）手術の中でも胸椎OPLLについてはいまだに手術成績不良例や術後悪化例が存在し，厚生労働省研究班の全国前向き研究では術後合併症率51％，（一過性を含む）術後運動麻痺率32％と報告され，非常に難易度が高い（図 1）^12）．当科では嘴状型胸椎OPLLに対し，後方除圧矯正固定術と後方進入脊髄前方除圧術（大塚変法であるRASPA法）を行っているので^{5,6,8,9,11,13）}，本稿では当科の2期的手術strategyと後方除圧矯正固定術の術式について概説する．
