　胸椎後縦靱帯骨化症は高度脊髄障害を呈することが多く，術後下肢麻痺のリスクが非常に高い．当科では，後方から広範囲椎弓形成術・椎弓切除術とインストゥルメントを併用した後弯矯正固定術を一期的に行う一期的後方除圧矯正固定術を行い良好な成績を挙げている．本稿では一期的後方除圧矯正固定術を行った37例の手術成績を，脊髄モニタリングとともに調査した．術後症状は退院以降も徐々に改善し，術後1～2年まで改善がみられた．術後回復傾向があれば必ずしも早期の前方除圧術追加は必要でなく，後弯矯正固定による間接的脊髄除圧も得られる本術式は，推奨できる手術方法の一つである．
