整形外科における内視鏡下手術の展望

Future of Endoscopic Surgery in Orthopaedics

　内視鏡下脊椎手術は，術式としてまだ発展段階にあり，一般内視鏡外科の手技を従来の脊椎手術に応用しているに過ぎない．手術時間に関しても従来法より多くかかっている手技が多く，最小侵襲手術を目指すのであればより効率よい手技の確立が急務とされる．今後さらに内視鏡脊椎外科専用の手術器具，脊椎インプラントの開発が望まれる．また将来には，X線イメージのみならず，MRIを術中に再現しながら脊柱の変形矯正や再建術も内視鏡下に行うことが可能であろう．より細径の内視鏡でより鮮明な画像が得られ，術野を確保できれば，脊椎外科の多くの手術手技を内視鏡的に行い得る．工学と医学の協力の下でロボティックス，テレサージェリーが進めば，これまでの脊椎手術の概念を越えた治療が行えるものと期待している．

Endoscopic spine surgery is now being developed using conventional endoscopic techniques, which have been developed in general surgery. Spine surgery requires special instruments including the air-tome for bone cutting and fixation devices for spinal fusion. However, these devices are inconvenient for endoscopic surery. New spinal instrumentation system is necessary for endoscopic surgery. The correction of spinal deformities requires special endoscopic techniques, and, we will be able to correct even rotational deform-ities using new instrumentation systems.
