形成外科における内視鏡下手術の展望

Latest Trends and Perspectives in the Field of Plastic and Reconstructive Surgery

　内視鏡下治療は，切開創の短縮による整容面の改善，術後疼痛や合併症の軽減など多くの恩恵をもたらしている．形成外科では頭蓋額顔面領域の整容外科や各種骨折の整復固定，乳房再建，腫瘍摘出術，漏斗胸手術など内視鏡下手術の対象は多岐にわたり，手術手技や内視鏡システム，周辺機器，微小血管吻合や血流モニタリングなどの分野でさらなる改善の努力がなされている．近い将来，先端技術の導入により，人間工学に基づいた内視鏡や手術支援システムの開発，モニタリングによる安全性の向上，高速通信ネットワークの導入といった周辺環境が整備され，より低侵襲で，より質の高いQOLをめざす医療の基本手技の1つとして確立されるものと思われる．

Endoscopic therapy has provided many benefits such as esthetic improvement by reducing operative wounds and reduction of postoperative pain and complications. In plastic and reconstructive surgery, indications for endoscopic surgery are diverse: esthetic surgery in the craniofacial region, reduction and fixation of various fractures, reconstructive mammaplasty, tumorectomy, surgery for pectus excavatus, and so forth. Efforts are being made to further improve surgical techniques, endoscopic systems, peripheral devices, microvascular anastomosis, and blood flow monitoring.
