腰椎変性疾患に対する内視鏡下腰椎前方椎体間固定術

Endoscopic anterior lumbar interbody fusion for degenerative lumbar diseases

　内視鏡下腰椎前方固定術が，真に低侵襲な手術手技になるには解決すべき問題点がある．本稿では，現在行われている内視鏡下腰椎前方固定術の術式の長期成績について報告し，またこの術式の問題点を示した．術前JOAスコアが平均14.4点であったものが術後平均27.6点，平均改善率88.8％と著明に改善していた．術後5年以上の経過をみた症例でも十分に満足できる成績を示したと判断できる．ただし固定法により経過に差が認められる．椎体間固定のためにはケージを2個使用するか，従来法と同じく腸骨を利用した骨移植を行うべきで，脊椎ケージ1個のみの使用法は慎重に行うべきである．今後さらに固定性のよい脊椎インプラントが開発されれば，除圧とともに即時的な固定性を獲得できる術式が完成するであろう．

Endoscopic anterior lumbar interbody fusion will be a true less invasive surgery for lumbar disc diseases, but still there are problems should be resolved. This paper reports the long term surgical results of this surgical technique and tried to clarify these problems. The average JOA score was 14.4 preoperatively, the score became 27.6 and the recover rate was 88.8%. This showed the excellent results even 5 years after surgery. However, there was a significant difference in long term surgical results between the cage group and iliac bone graft group.
