内視鏡下脊椎手術における超音波凝固切開装置の応用

Clinical Application of Ultrasonically Activated Instrument for Endoscopic Spine Surgery

　内視鏡下脊椎手術に対して超音波凝固切開装置を使用した経験を報告し，その利点と問題点について述べた．電気メスを用いたこれまでの止血操作と異なり凝固，切開能に優れ，特に血管閉塞能に優れていた．脊椎外科領域においては椎体の周辺の血管の処理に有用であり，また椎間板の切離にも使用可能である．神経組織近傍での剥離，止血，切開操作を安全に行うことが可能である，一方，鉗子の形状が限られる点，またグリップなどに操作性の問題が残り，今後より使いやすい形状の超音波凝固切開装置が開発されることが望まれる．

We reported the effectiveness of the ultrasonically activated device (USAD) for endoscopic spine surgery. This device is useful in such techniques as coagulation and dividing, separating tissues with very little amount of bleeding under laparoscopy. The lumabr segmental vessels are clipped safely after the coagula-tion of small vessels using this USAD. The lumbar intervertebral disc is excised effectively before the curretage by a rongeur. Safe separation of the neural tissue is possible, in case of neurinoma from the neural foramen.
