小児外科における内視鏡下手術の展望

Future Perspective for Endoscopic Surgery in Children

　成人外科に遅れて導入された小児外科領域における内視鏡下手術は，いまだ発展途上にあるものの，小児用器具の開発に伴い，ここ数年における適応疾患の拡大には目覚ましいものがある．いくつかの疾患では内視鏡下手術が第一選択とされ，標準術式になりつつある．21世紀には，小児内視鏡下手術が普遍的となり，小児外科領域のほとんどすべての疾患が，補助的使用も含めた内視鏡下手術の適応疾患となるであろう．問題は，適応疾患の拡大に伴う小児外科医のトレーニングをいかに行うかであり，このトレーニングシステムの確立が，21世紀の小児内視鏡下手術，ひいては小児外科そのものの発展につながると考える．

The use of endosurgery in children has lagged behind its use in adults. However, in recent years, with the development of specialized instrumentation, the indications for endosurgery in children have increased remarkably, and it is now regarded as the treatment of choice for several conditions. In the 21st century, the use of endosurgery will be even more extensive, and virtually all surgical procedures in children may one day be performed using endosurgery.
