内視鏡下腰椎前方固定術

Endoscopic Anterior Lumbar Interbody Fusion

　腰椎前方固定に内視鏡下手術を応用した手術手技の開発を行ってきた．腰椎固定専用の手術器具を作成し，効率よい除圧と脊椎固定が行えることがわかった．特に，効率よい手術操作のために脊椎シースを作成したところ，これを利用することによって術野が容易に確保でき，従来の手術器具も使用可能となった．シース壁には内視鏡が固定でき，明るい視野が確保され，安全に椎間板切除や椎体固定が可能であった．骨切除に際しても気腹が必要ないため，ガス塞栓の危険もなく器具も簡略化できた．椎体間固定のみが適応となる症例では，即時的な固定性を有する脊椎内固定材（spi-nal fusion cage）を利用した術式が，また，ヘルニア摘出など椎間板後方を詳細に観察し神経組織の除圧を必要とする症例には，本術式が適応と考える．

The intervertebral space can be exposed and well visualized using either the transperitoneal or the extraperitoneal approach. Even through retroperitoneoscopic procedure, a relatively large bone graft with sufficient depth placement, can be performed. The intervertebral space can be exposed safely and well visualized using a spinal sheath which was developed by ourselves. Using this spinal sheath, gasless surgery is possible during the vertebral osteotomy. Posterior part of the disc space was well visualized and the nervous tissues were safely and completely decompressed.
