腹部大動脈瘤手術における内視鏡下手術の臨床応用

Clinical Application of Retroperitoneoscopic Techniques to the Operation of Abdominal Aortic Aneurism

　内視鏡下手術の浸透は，一般外科領域では急速であったが，わが国の血管外科領域ではまだ始まったばかりである．腹部大動脈瘤手術は大きな侵襲を伴い，改善が望まれる．動物実験をもとに，後腹膜腔鏡の腹部大動脈瘤手術への応用を，わが国初の症例を含む3症例（腎動脈下，φ約5〜6cm）で経験した．内視鏡外科医が左後腹膜腔から腹部大動脈露出，下腸間膜動脈処理，腰動脈結紮・切離，大動脈遮断を行う．そのうえで，血管外科医が腹部正中の小開腹（6〜12cm）で直視下に人工血管置換術を施行した．良好な視野で，安全かつ繊細な手術が可能であった．また，専門医との連携で高度な治療が可能であった．本法は今後の腹部大動脈瘤人工血管置換術の治療法の1つとなり得る手技と考えられる．

Since the advent of laparoscopy, the sweeping changes in vascular surgery are seen and have just begun in our country. The operation of abdominal aortic aneurysm (AAA) is too invasive and a new technique that can be offered as less invasive is hoped for. On the basis of our previous animal experience with laparoscopic vascu-lar surgery, we have examined the application of retroperitonealscopic techniques to intraabdominal vascular procedures in clinical practice. We have experienced the first three laparoscopy assisted abdominal aortic aneu-rysm repair cases in Japan.
