HALSにおける器具の進歩

Progress in the Device of Hand-assisted Laparoscopic Surgery

　腹腔鏡下手術におけるハンドアシスト法（HALS）は，術者の手を腹腔内に挿入できる手術方法であり，ここ数年でにわかに注目され始めている．本邦ではLAP DISC^®®，Handport^®，Pneumosleeve^®などの市販品も含め，いくつかのHALS用器具が考案されている．いずれも気腹下でのHALS手技を確実に行えることを目的とするが，サイズも使用方法も様々である．本稿ではこれらの各システムを概観し，それぞれの特徴をもとに安全性，経済性，応用可能性などにつき比較検討した．今後HALSは手術適応の拡大や合併症の減少，新規術式の導入など，多くの面で腹腔鏡下手術に貢献することが期待される．

Hand-assisted laparoscopic surgery has started to be recognized as a new feasible technique for introducing surgeon's hand into abdominal cavity during laparoscopic surgery. Several systems for HALS are now available in JAPAN, including three commercial products (LAP DISC^®, Handport^® and Pneumosleeve^®). All of these systems are developed to secure pneumoperitoneum while surgeon's hand works in the abdomi-nal cavity, retracting organs and controlling surgical procedures just like in open surgery.
