脾臓摘出術におけるHALS—“人工肛門用パウチ”を利用して

Hand-assisted Laparoscopic Surgery in Splenectomy Using Enterostomal Adhesive Flange and Pouch

　教室では，特発性血小板減少性紫斑病（ITP）7例に対して，“人工肛門用パウチ”を利用したhand-assisted laparoscopic splenectomy（以下，本法）を施行した．本法は，人工肛門用パウチを利用してair-tightとし，気腹下に内視鏡下手術を施行する方法である．脾臓摘出の場合，約7cmの皮切をおいて術者の左手を腹腔内に挿入し，他に腹腔鏡用と操作用の2本のトロッカーを要する．トロッカーの数と場所を変えることでどんなHALSにも応用可能である．人工肛門用パウチを利用することにより，HALSは非常に簡便・容易に，しかも安価にできる方法となる．

Laparoscopic surgery has rapidly been applied to many intraabdominal surgeries because of the excellent result of the cholecystectomy. However, lack of tactile sensation and precise manipulation of organs were biggest disadvantage of this procedure. Hand-assisted laparoscopic surgery (HALS) is the surgical technique that solves these disadvantages of endoscopic surgeries. Here we present a procedure of the hand-assisted laparoscopic splenectomy using enterostomal adhesive flange and pouch instead of the HALS device such as Pneumo Sleeve^TM or LAP DISC^TM to reduce surgical cost.
