LAP DISC（再気腹用腹壁ディスク）を使用した新しいハンドアシスト法腹腔鏡下手術

A New Hand-assisted Laparoscopic Surgery using “LAP DISC”

　腹腔鏡下手術は，2次元画像のもとでの遠隔操作というハンディキャップをもっている．これを改善する目的で，われわれはハンドアシスト法に用いる腹壁ディスク（LAP DISC）を開発した．この装置は，気腹を維持したままで術者の手を腹腔内に挿入し，術中の触診や，臓器の把持，牽引などを行うことができる．したがって，比較的標本の大きな手術，悪性疾患に対する手術，術中に出血などの合併症発生の可能性が比較的高い手術など，広い範囲の腹腔鏡下術式に応用が可能である．LAP DISCを用いたハンドアシスト法の腹腔鏡下手術は，これまでの腹腔鏡下手術術式の安全性，確実性の向上，手技の平易化，適応の拡大に大きく寄与する方法と考えられる．

Laparoscopic surgery is a procedure reformed without the use of tactile sensation. We newly invented a abdominal wall disc “LAP DISC” for hand-assisted laparoscopic surgery to reduce such disadvantage. “LAP DISC” enable surgeon to insert his hand into abdominal cavity for examination and retraction of organs even while maintaining pneumoperitoneum. The method is applicable for laparoscopic procedures with a certain size of resected specimen, with malignant disease, or with highly possible complications such as intraoper-ative massive bleeding.
