内視鏡下食道癌切除術におけるハーモニック・スカルペルの有用性

Usefulness of Harmonic Scalpel in Thoracoscopic Esophagectomy for Esophageal Cancer

　内視鏡下胸部食道癌切除術におけるハーモニック・スカルペルの有用性について検討した．胃管作成，経食道裂孔—後縦隔郭清および胸膜の切開，食道周囲の剥離，縦隔リンパ節郭清に際し，shears型を用いて凝固・止血を行った．ハーモニック・スカルペル使用5例の胸腔内操作時間は125.0±28.7分，出血量は134.8±70.2mlで，未使用8例の各々106.4±42.9分，250±107.8mlに比べて手術時間は長くなる傾向を示したが，出血量は有意に減少した（p＜0.05）．ハーモニック・スカルペルは，周囲の組織を熱損傷することなく，安全で確実な止血と良好な視野が確保できた．

We used a harmonic scalpel in thoracoscopic esophagectomy for esophageal cancer to coagulate and cut the small vessels and tissues. The harmonic scalpel-shears type was effective for preparation of the omentum for a gastric tube and deviding and dissecting the thoracic esophagus and mediastinal lymph nodes. Loss of blood was significantly less in the patients when the harmonic scalpel was used than when it was not used (134.8 ± 70.2 ml vs. 250 ± 107.8 ml, p<0.05).
