ハーモニック スカルペルを利用した内視鏡補助下乳腺部分切除術

Endoscope Assisted Partial Mastectomy with Harmonic Scalpel

　近年，乳房温存療法を施行する機会が年々増加している．しかしコスメティックな点からすると，残存乳房に皮膚切開線が残らないほうが好ましいのは当然である．われわれは腋窩に小切開を入れ，内視鏡補助下にての乳腺部分切除術および腋窩リンパ節郭清術を行い，美容的に満足の得られた症例を経験している，内視鏡補助下乳腺部分切除術は遠隔操作であるため，確実な止血および正確な切開を必要とするが，ハーモニック　スカルペルを利用することでこの問題点は解決される．われわれの行っているハーモニック　スカルペルを利用した内視鏡補助下乳腺部分切除術の手技を報告した．

Recently, breast conserving treatment has been performed more and more. There is a cosmetic disadvan-tage when a skin incision is made on the conserved breast. So we have applied endoscopic treatment for breast cancer to resolve this problem. Utilization of Harmonic scalpel results in an excellent hemostasis and a facilitating dissection. Thus it is a good choice to use Harmonic scalpel in endoscope assisted partial mastectomy. The techniques of our procedure are described.
