Utilization of the Harmonic Scalpel in Conventional Surgeries

　ハーモニック・スカルペルは，内視鏡下手術の発展に伴い使用される機会が増えているが，その特徴は優れた止血効果とより小さな組織損傷である．特にlaparosonic coagulating　shears（LCS）は，血管を結紮処理することなく切離可能であり，その有用性は従来の開胸・開腹手術においても認められつつある．特に直腸手術などの狭く深い術野での血管処理や組織剥離を伴った手術，膵組織の切離などの際には非常に有用であると考えられる．ただし開腹手術においてさらに普及するためには，いくつかの問題点があることも事実であり，今後の改良が望まれる．

Utilization of the Harmonic Scalpel results in a excellent hemostasis and a less thermal damage of adjacent tissues. Especially, LaparoSonic Coagulating Shears (LCS) provides safe dissection of the vessels without any ligation or clipping. This instrument is considered very useful also in a conventional surgery, as well as in an endoscopic surgery. Surgery for the rectum which requires dissection in a narrow area, is considered a good indication for utilization of this instrument.
