ハーモニック・スカルペル*は超音波振動のエネルギーを利用した手術装置で,従来の超音波メスとは異なり,組織の破砕・吸引ではなく凝固・切開を行うことを目的とした器械である.電気メスやレーザーと比較して低温で凝固作用が発生し,側方の熱損傷が非常に少ない.また,組織の炭化に伴う煙の発生がなく,患者に通電することもないというこれらの特徴は,内視鏡下手術のみならず開腹・開胸手術においても非常に有効である.本稿では,これらの特徴と利点がどのような原理で得られるかについて,器械的な観点から述べた.
Harmonic Scalpel (HS) is a surgical device utilizing ultrasonic energy to cut and coagulate tissues, different from conventional Ultrasonic Cavitational Aspirator (UCA) which is designated to fracture and aspirate soft tissues. In comparison with other energy sources such as electric surgery or laser surgical unit, the system coagulates tissues with relatively low heat and limited lateral thermal injury without charring the tissues. It also generates no smoke and the patient is completely isolated from the electrical circuit. These features of HS are considered very effective not only in endoscopic surgery but also in open surgery.
