腹腔鏡下脾臓摘出術におけるハーモニック・スカルペル使用法のコツ

Laparoscopic Splenectomy with Harmonic Scalpel

　腹腔鏡下脾臓摘出術は，従来の開腹下脾臓摘出術の適応疾患すべてが手術適応となり，保険診療も認められ，いまや標準術式となっている．脾摘術においては，ハーモニック・スカルペルは，脾門部の血管処理を除くすべての剥離操作，すなわち脾結腸間膜，脾腎ヒダ，横隔脾ヒダ，胃脾間膜の剥離および切離や小血管の止血，切離に使用可能である．特に門脈圧亢進症や出血傾向のある症例では，残す側にクリップをかけることで確実な止血操作が可能である．一方，脾動静脈および門脈圧亢進症がある場合の胃脾間膜の切離は，自動吻合器が有用である．脾臓の摘出は，モルセレーターを用いると皮切を延長することなく摘出可能である．

Laparoscopic splenectomy is now in world wide use. Harmonic scalpel, which is an ultrasonic coagulating dissector, was very useful for the resection of splenocolic, phrenocolic, splenorenal, gastrosplenic, spleno-phrenic ligaments, except in patients with portal hypertension. Linear Cutter was effectively used in the resection of splenic artery and vein at the splenic hilus. The resected spleen was inserted in a sac, cut into small pieces and taken out of the intraperitoneal cavity together with the sac.
