We examine characteristics and differences between foreign and Japanese guidelines for clinical nutrition in critical care. These guidelines were created using different methodology. There were very few high quality studies and were few studies that show significant clinically important differences in parameters such as survival rates. Therefore, it is hard to present clear recommendations. The Japanese guidelines present recommendations for interventions in Japanese medical practice. Since there were few high quality studies, some clinical situations do not have clear recommendations. It is necessary to conduct a high-quality multicenter study to solve the unsolved problems that became clear in the creation of these guidelines.
