Guidelines for clinical nutrition in critical care

海外ならびに本邦の重症患者における栄養ガイドラインの特徴と相違について検討した。作成方法はそれぞれ異なってはいるうえ，栄養療法に関する研究には質の高い研究が非常に少なく，また，症例数が少ないこともあり，臨床的に意義のある生存率などで有意差を示し得た研究も少なく，明確な推奨を提示しにくい現状があることが明らかとなった。日本のガイドラインは，日本で行われている介入や現状を加味しており参考となるが，質の高い研究が欠けるため，やはり明確な推奨に至らないものが少なからず存在した。

　今後は，これらのガイドラインで明らかになった未解決の問題点を解明するために，質の高い多施設研究を行う必要がある。

We examine characteristics and differences between foreign and Japanese guidelines for clinical nutrition in critical care. These guidelines were created using different methodology. There were very few high quality studies and were few studies that show significant clinically important differences in parameters such as survival rates. Therefore, it is hard to present clear recommendations. The Japanese guidelines present recommendations for interventions in Japanese medical practice. Since there were few high quality studies, some clinical situations do not have clear recommendations. It is necessary to conduct a high-quality multicenter study to solve the unsolved problems that became clear in the creation of these guidelines.
