Guidelines for nutrition support of critically ill patients are available from various organizations, such as SCCM/ASPEN, ESPEN, Canadian Critical Care Clinical Practice Guidelines Committee, Asia Pacific and Middle East Working Group on Nutrition in the ICU, and the Japanese Society of Intensive Care Medicine. It would be desirable to have similar recommendations whatever organization they are from, but they are not. Despite similar clinical questions, variations remain among the guidelines due to differences in year of publication and interpretation by each organization. These variations can cause confusion in the clinical setting. To avoid this and to make appropriate interventions, it is necessary to understand the key differences and reasons behind the differences. The aim of this review is to enhance appropriate decision making in the clinical setting by outlining the differences among the guidelines and as yet unexplored areas of nutrition therapy for critically ill patients.
