Dual control ventilation (DCV) is a ventilator mode developed to integrate the characteristics of volume control ventilation (a specified tidal volume and minute ventilation) with those of pressure control ventilation (variable inspiratory flow and reduced work of breathing, better patient-ventilator synchrony). Pressure regulated volume control (PRVC) is a type of DCV, which allows the ventilator to achieve the volume target with a pressure-controlled breath under closed-loop control of the pressure, comparing the set and delivered tidal volumes. Clinicians may believe that PRVC is superior to traditional ventilator modes following the “kill-two-birds-with-one-stone” concept and easily set parameters. However, this self-adjusting mechanism may be a major weakness by which the ventilator cannot distinguish improved pulmonary compliance from increased patient respiratory effort. There are no high-quality data showing that PRVC is superior to traditional modes. Clinicians should be aware of advantages, limitations, and appropriate indications for PRVC.
