Chylothorax is an accumulation of chyle in the pleural cavity. Most chylothoraces are caused by surgical trauma as a complication of thoracic or mediastinal procedures. It is an uncommon, but challenging complication associated with significant morbidity and mortality if left untreated. Since chyle contains fat and other nutrients absorbed by the intestine, chyle leakage causes nutritional complications.

 Nutritional therapy plays an important role in the non-operative management of patients with chylothorax. Clinicians often use nutritional approaches such as a low-fat diet or parenteral nutrition which are based on the physiology of chyle production. However, most of these approaches are supported not by definitive evidence, but by clinical experience. Existing studies are mainly small observational trials and case reports.
