慢性中耳炎と内耳障害

CHRONIC OTITIS MEDIA AND INNER EAR LESIONS

　臨床的に慢性中耳炎，中耳真珠腫症として診断されたものに聴力検査を行なつてみると，高音の低下したものがあつたり，骨導の低下しているものがある。古くは高音低下は内耳障害と考えられていたのであるが，臨床と実験からして伝音系障害でも高音低下することのあることが明らかとなつた。また骨導の低下は内耳障害を示すと考えられていたが，耳硬化症におけるCarhart notchの存在^1）と実験的結果^6）からして伝音障害においても骨導が15〜20dB低下することのあることが明らかとなつた。こうした現状において慢性中耳炎に認められる内耳障害の問題について検討した。

　The present status of chronic otitis media that forms the basis of inner ear lesion is studied seeking the means of their prevention.

　The loss of high-tone hearing and shortening of bone-conduction as manifestations of the inner ear lesion are discussed and their corresponding incidences are noted.

　There were 69 cases of inner ear lesion among 247 cases of cholesteatoma (28%) and similarly, 71 cases among 426 cases of chronic otitis media (18%)

　The study advanced to discussion on the mechanism of occurrence of inner ear lesion in case of cholesteatoma and chronic otitis media.

　Thus the efforts were made to find the preventing means for such sequelae.
