鼓室成形術による内耳障害

COCHLEAR LESIONS FOLLOWING TYMPANOPLASTY

　慢性中耳炎の手術的治療においては中耳内の炎症と炎症性病変とを除去し，破壊された伝音系を再建する。これによつて耳漏を消失せしめるとともに，失われた聴力を回復せしめんとしている。

　ところが本手術によつて内耳障害を引き起こし目的に反して聴力低下することがあり，めまいを訴えることがある。しかしてめまい症状は特殊な場合を除いて時日の経過とともに回復するものであるから，永久障害とはならない。これに反し術後の難聴は永続障害となるものが多く，しかも聾となることがある。このような手術に関連して起こつた蝸牛障害は手術の目的とまつたく反した結果であり，不幸なことである。したがつてこの問題を解決することは臨床的にきわめて重要である。

　The bone conduction acuity of 874 cases who had tympanoplasty was reported. The bone conduction acuity decreased postoperatively in about 10% of the subjects with better preoperative bone-conduction threshold and in about 16% of those with less preoperative acuity. This tells us that the operation of the patients with mixed type hearing loss should be carried out with special care. The postoperative loss in the bone conduction acuity was mostly found in those whose tympanums showed severe pathological changes. From the long-term observation it was found that some cases increased, while some decreased, the bone conduction acuity after tympanoplasty.
