中耳の手術について

ON THE MIDDLE EAR OPERATION

　I．はじめに

　ここにこのような題をえらんだのは，最近における中耳の手術がすつかり変貌したことが第一である。そのためにともすれば誤つた考えを持つているのではないかと思われる発表があることてある。またそのために起こつた名称の混乱があつて，名称のために特に異なつた手術法であるかの錯覚にも陥る恐れがあるからであり，またこうしたことから恐らく多くの方が困惑していると考えたからである。第二の大きな理由は中耳の病態病変が第1表に示すように，従来あまり考えられていなかつたものがクローズアップされてきたことも中耳の手術において変化をきたした一因である。第三の大きな理由に手術に対する考え方が炎症消失から機能回復へと主力が注がれてきたことがあり，第四の理由にそれに伴つて手術が再建的なものに変遷してきたのは当然であるが，第五に病態がいろいろであるためにいろいろの手術法がある訳であり，それを新しい手術法として適応を明示せずして発表しているための混乱がみられるからである。

　Since tympanoplasty has entered its new phase of recognition various names have been applied to different types of operation as if they were some new methods. But, in the last analysis, all of these proved to be identically similar to what have been, in the past, included under the term of modified radical mastoidectomy.

　The author considers the importance of the middle ear ossicular chain, not only from the standpoint of the preservation of sound conduction but, also, as a support to the tympanic membrane that maintains the important intact epitympanic space.

　Two different approaches to the care of incudo-stapedial joint are described; one through the mastoid cavity and the other through the attic.
