慢性中耳炎による迷路瘻孔と内耳機能

Postoperative Course of Inner Ear Function in Labyrinthine Fistula

I．はじめに

　慢性中耳炎において炎症が内耳へ波及する経路としては，正円窓，卵円窓，迷路瘻孔が考えられる。これらのうち迷路瘻孔が内耳へ与える影響については種々の報告がなされてきた。慢性中耳炎の手術の際，迷路瘻孔が見られることは稀ではなく，最近では，Gacck^1）の8.3％，Abramson^2）の6.6％，斉藤^3）の8.2％が報告されている。真珠腫性中耳炎については，立木^4）は10.4％，Shcchy^5）は9.5％に瘻孔が見られたと述べている。

　今回我々は，手術の際，迷路瘻孔を認めた18例について，臨床所見および術前術後の内耳機能の経過を比較検討したので報告した。

　Clinical findings of preoperative and postoperative inner ear function were analysed in 18 cases of labyrinthine fistula which were found at the operation for chronic otitis media. In the fistula cases, vertigo (77.8%), spontaneous nystagmus (77.8%), sensorineural hearing impairment (94.4%), and decreased caloric response (100%) were more often than fistula symptom (44.4%). Postoperative impairment of bone conduction of 15 dB or more occurred in 3 cases (18.8%), whereas deterioration developed in 3 cases (18.8),). After postoperative long term observation, caloric response was improved in 4 cases (28.6%).
