 The impalement injury of head and neck is extremely rare. We reported a 69-year-old man with impalement injury by a wooden stick. He fell from a motorcycle and jumped into a hedge. A wooden branch stuck his left cheek and penetrated his right neck to the oral cavity. Contrast enhanced CT showed that a wooden branch passed near the common carotid artery and internal jugular vein. The wooden stick was successfully removed by transcervical approach following tracheostomy without severe sequelae. Preoperative CT was useful to know the positional relationship between a foreign body and the important organs.
