鼻内内視鏡手術における下鼻道アプローチの有用性

Usefulness of the approach in the inferior nasal meatus in nasal endoscopic surgery（ESS）

Ⅰ．はじめに

　炎症性疾患，腫瘍性疾患を問わず，鼻・副鼻腔疾患に対する外科的アプローチにおいて，鼻内内視鏡手術（endoscopic sinus surgery：ESS）は重要な位置を占めている。さまざまな機器の発達により，鼻・副鼻腔のほとんどの部位がESS下の操作で行えるようになっているが，膜様部を介しての操作では，上顎洞内すべての部位が操作できるわけではない。特に上顎洞の前方，上顎洞底は，膜様部からの内視鏡操作では手術操作が困難である。そこでわれわれはESSにおいて上顎洞前方，上顎洞底の操作ができるように，必要に応じて下鼻道から上顎洞を開放する下鼻道アプローチを行っている。

　ESS by the inferior meatus approach was performed for 8 cases；2 cases of fungal sinusitis, 2 cases of maxillary papilloma, and each one of maxillary tumor blood reserve, maxillary hemangioma, sinusitis, and retension cyst. In all cases, dye was injected through the tear duct, and after detecting the opening of the nasolacrimal duct using endoscopy, the maxillary sinus was accessed from the inferior meatus. The lesion located in the anterior part and floor of maxill sinus was removed by the approach through the membrane and the inferior meatus.
