We observed the foveal sparing phenomenon ofpreretinal hemorrhage which involved maculararea in 18 eyes of 48 eyes (38%). Since preretinalhemorrhage is a clinical term which implies sub-hyaloid or subinterlimiting membrane hemorrhagein anatomical aspect, We intended to identify theanatomical location of preretinal hemorrhage withfoveal sparing.

We examined all of 18 eyes with foveal sparingby slit-lamp using Goldmann's three mirror contactlens. We could observe the vitreous directly byintraocular illumination in 13 eyes which wereoperated for vitrectomy because of subsequentvitreous hemorrhage. These results confirmed sub-hyaloid nature of these hemorrhages. Clinical evo-lution was observed at least 6 months to 3 years inall of 18 eyes which included 16 eyes of diabeticretinopathy and 2 eyes of retinal branch vein occlu-sion. Their clinical course suggested that sub-hyaloid hemorrhage occurred at initial stage ofposterior vitreous detachment.

We concluded that the foveal sparing phenome-non is a clinical sign which assures subhyaloidnature of the preretinal hemorrhage.The fovealsparing phenomenon of preretinal hemorrhageseems to reflect strong vitreoretinal attachment atthe fovea.
