中心窩白色顆粒症

White dot fovea

　従来文献に記載のない中心窩の変化を発見した。中心窩の表面またはその縁に白色の顆粒が多数分布することがこの状態の中軸所見であり，これを仮に中心窩白色顆粒症と命名した。過去4年間に偽黄斑円孔として検索した症例のうち，48眼25症例にこれが観察された。年齢は53〜73歳，平均64歳であり，男子9例17眼，女子16例31眼であった。中心窩白色顆粒症は，ときに黄斑円孔に似た所見を呈するが，変視症や視力障害などの自覚症状が皆無であることと，他覚的にも顆粒以外には著変がなく，無害であることがその特徴であった。顆粒の数は100〜300個の範囲であり，その大きさは直径約5μmであった。以前に行った剖検眼の網膜表面の走査電顕による検索で，中心窩白色顆粒に相当すると推定される所見が，30眼中5眼に観察された。走査電顕での顆粒は，数が100〜200個であり，その大きさは5〜20μmであった。部位は内境界膜よりも内方にあり，その形態から，グリア由来であると判断された。臨床的に観察された白色顆粒と走査電顕での顆粒の大きさの違いは，これが半透明であるために，検眼鏡または走査レーザー検眼鏡による検索では，実際よりも小さく見えることがその原因であると考えられた。中心窩白色顆粒症は，乳頭付近の網膜に生じるGunn斑と似た性質であると推定された。本症は偶発的に発見されるのが通例であり，実際の頻度はかなり高いこと，そして，特発性黄斑円孔の鑑別診断で考慮されるべき一つの状態であることが強調される。

We identified a new foveal lesion mimicking idiopathic macular hole in 48 eyes of 25 cases. The foveal lesion was characterized by the presence of numerous white granules on the surface of foveal depression. The white granules were distributed either diffusely or along the foveal margin forming a grayish ring. We tentatively named this condition as 'white dot fovea'. The cases included 9 men and 16 women. The ages ranged from 53 to 73 years, mean 64 years. The condition was bilateral in all cases except two. There were no subjective symptoms nor visual disturbances. The condition was best seen by a scanning laser ophthalmoscopy using argon blue laser as the light source. The white granules numbered from 100 to 300 per eye. Each granule was about 5 μm in diameter.

In our earlier observation by scanning electron microscopy in autopsy eyes, foveal granules simulating the white dots were observed in 5 of 30 senile eyes. The number of the granules ranged from 100 to 200 and its diameter from 5 to 20 μm. The granules had multiple protrusions with cilia, which resembled glial cells. We postulate this glia-like structure in autopsy eyes as a couterpart of clinically observed 'white dot fovea'.

Since white dot fovea has been found by chance observation, its actual incidence seems to be high. This new clinical entity merits due attention in the differential diagnosis of macular holes.
