We examined the vitreous in 62 eyes with idiopathic preretinal macular fibrosis using slit-lamp biomicroscope and Goldmann three-mirror lens. All the eyes manifested complete posterior vitreous detachment. We identified an oval-shaped defect of the detached posterior hyaloid membrane in all the eyes corresponding to the size and extent of the premacular membrane.

The oval-shaped window in the cortical vitreouswas always sharply demarcated with collagen fibers arrayed along its margin. Either a liquefied space was present anterior to the window, or the vitreous gel prolapsed posteriorly through the window.

These findings seemed to indicate that an oval -shaped remnant of the posterior hyaloid mem-brane is a component of the premacular membrane and that the defect in the detached cortical vitreous corresponds to the base of the bursa premacularis.
