We treated 4 eyes in 2 cases of juvenile retinoschisis complicated by macular detachment. The first case was a 34-year-old woman. Both eyes had myopia of -7.5D. Her visual acuity was 0.5 right and 1.0 left. It decreased to 0.1 right and 0.5 left 10 years later. Optical coherence tomography (OCT) showed reti-noschisis and retinal detachment in the macula. Her right eye received intravitreal injection of SF6. The vitre-ous cortex adherent to the fovea became detached. Retinoschisis and macular detachment disappeared. Her left eye was similarly treated twice. The inner fovea became detached together with the viteous cortex, re-sulting in lamellar hole and later in full-thickness macular hole. The visual acuity improved to 0.9. The other case was a 26-year-old man. Both eyes had myopia of -3.5D. His visual acuity was 0.3 right and 0.9 left. The right eye received vitrectomy and gas tamponade using SF6. The visual acuity improved to 0.9 but de-creased to 0.1 due to macular atrophy. His left eye developed macular detachment 7 years later. The visual acuity decreased to 0.5. Vitrectomy and gas tamponade induced disappearance of retinoschisis and detach-ment in the macula. The visual acuity improved to 0.8. Vitreous traction to the fovea appeared to have played a key role in the development of foveal retinoschisis and macular detachment in both cases.
