　黄斑円孔のない後部ぶどう腫内の網膜剥離3例4眼に対して硝子体手術を行った。すべて女性であり，年齢は36歳から62歳で，−11Dから−18Dの近視が罹患眼にあった。光干渉断層計OCTで中心窩網膜剥離と中心窩周囲の網膜分離が4眼すべてにあった。黄斑部の残存硝子体皮質除去と内境界膜剥離を3眼に行った。網膜剥離と網膜分離は以後軽快し、視力が改善した。残存硝子体皮質除去が不完全であった他の1眼は，術後に黄斑円孔が生じた。以上の所見から，強度近視眼での中心窩網膜剥離と網膜分離が黄斑円孔網膜剥離の前駆病変でありうることと，この段階で硝子体牽引を除去することにより黄斑円孔化を予防し，視力改善が可能であると結論される。

We performed vitreous surgery on 4 eyes of 3 patients. All the 4 eyes had myopia ranging from -11 to -18 diopters, posterior staphyloma, and no macular hole. All were females aged from 36 to 62 years. Optical coherence tomography showed retinal detachment and retinoschisis in the fovea in all the 4 eyes. We removed premacular vitreous cortex and internal limiting membrane in 3 eyes. All the 3 eyes showed gradual resolution of retinal detachment and retinoschisis followed by visual improvement. In the other one eye, removal of premacular vitreous cortex was incomplete and macular hole developed later. We postulate that foveal retinal detachment and retinoschisis may be precursors of macular hole retinal detachment in high myopia. Surgical removal of premacular vitreous cortex and internal limiting membrane is a treatment of choice in such cases.
