外傷性黄斑円孔の臨床像

Clinical features of traumatic macular hole

　外傷性黄斑円孔23例23眼の臨床像を検索し，特発性円孔と比較した。若年男子に多く，18例が，野球，サッカーボールによる打撲で占められていた。視力は，6眼で0.3以上であった。円孔が小さい場合と分層円孔では視力は良好であった。

　外傷性脈絡膜萎縮が，23眼中16眼に併発し，うち12眼は，三角症候群の形態を呈していた。黄斑に萎縮が及んだ10眼では，視力は0.1以下であった。隅角解離は7眼に，周辺部網膜裂孔は4眼に併発した。

　円孔は，新鮮例では，特発性に比べて小さく，その縁が不整で，正円ではなかった。円孔外縁の網膜の下掘れと縁の挙上はなく，周囲網膜は比較的平担であった。特発性円孔では，円孔外縁の下掘れと周囲網膜の浮腫が必発であり，際立った対照を示していた。硝子体検査を実施した13眼中12眼（全体の52％）で，後部硝子体剥離を認めた。一方，特発性円孔の新鮮例では，硝子体剥離はなかった。鈍的外傷による急性に生じた後部硝子体剥離により，中心窩に裂隙を生じることが外傷性黄斑円孔の主因であると推論された。

We evaluated the clinical features of 23 eyes withmacular hole secondary to blunt trauma. Chorior-etinal atrophy was also present in 16 eyes. Trau-matic macular holes were characterized by irregu-lar rugged margin, absence of undermining aroundthe hole and, when new, smaller size than idiopathicmacular holes.

Posterior vitreous was detached in traumaticmacular holes. In sharp contrast, posterior vitreousdetachment was consistently absent in idiopathicmacular holes in their early stage.

We conclude that acute posterior vitreous detach-ment during the impact is the key factor in thepathogenesis of traumatic macular hole.
