散瞳不良眼に対する眼内レンズ挿入術

Intraocular lens implantation surgery in miotic eyes

　瞳孔が十分に散瞳しない症例は，核の娩出が難しいため術中合併症を起こしやすく，眼内レンズを確実に嚢内固定することも難しい。そこで，このような症例に対して初心者が安全かつ容易に眼内レンズを挿入できるようにするため，intercapsular法による計画的嚢外摘出術を採用し，瞳孔径の大きさに応じて安全な核娩出法を工夫した。この方法を用いて行われた瞳孔径4mm以下の23眼では，術中合併症は少なく，重篤な術後合併症もなく，良好な術後視力が得られた。散瞳不良眼に対する本術式は，難しい手技がなく，初心者にとっても安全なものと思われる。

We developed an inercapsular procedure forplanned extracapsular cataract extraction to facili-tate extraction of lens nucleus and insertion ofposterior chamber intraocular lens. We applied thisprocedure to 23 eyes with miotic pupil of 4 mm orless during the foregoing 2 years. The findings weregratifying with fair final visual acuity. The proce-dure was free of complications during or aftersurgery.
