後房レンズ毛様溝固定の予後

Outcome of sulcus-fixated intraocular lenses

　最近，全摘後の無水晶体眼や術中後嚢破損例に対して，後房レンズ毛様溝固定が試みられるようになってきた。当院で最近約2年間に手術した18例21眼の術中術後経過について検討を行った。観察期間は平均6.3か月である。全摘出後5眼，嚢外後4眼，眼内レンズ偏位修正7眼などの二次手術計16眼の他，水晶体脱臼などによる一次手術計5眼を対象とした。前部硝子体切除後，粘弾性物質にて角膜内皮と虹彩を保護したうえで，垂直位に9-0プロリン糸を用いて毛様溝縫着固定した。ほとんどの症例で視力改善を認め，手術の合併症はいずれも軽微なものであった。また手術後の眼圧上昇は少なかったものの，房水流出率は低下していた。以上より，後房レンズ毛様溝縫着手術は視力予後がよく，比較的安全な手技と思われるが，血液房水柵への影響は否定できず，長期の経過観察が必要と思われた。

We treated 21 aphakic eyes without capsularsupport with transscleral cilicary sulcus fixation ofposterior chamber intraocular lenses. The patientswere followed up for 3 to 17 months, average 6.3months. As a standard procedure, anteriorvitrectomy was followed by suture of haptics to theciliary sulcus with a 9-0 prolene suture. The visualacuity improved after surgery in all the cases.Complications were absent except slight increase inintraocular pressure in a few instances.
